Erection changes during testosterone treatment need an individual review. They do not establish that the dose is wrong, that testosterone is the only contributor or that another medicine should automatically be added. Start by identifying what has changed and what treatment was intended to address.

Separate desire, erections and diagnosis
Describe whether the concern is sexual interest, obtaining an erection, maintaining one, pain or another change. These outcomes can require different questions. A symptom list or an isolated laboratory result cannot by itself explain what is happening.
The Endocrine Society's 2018 guideline bases diagnosis of male hypogonadism on compatible symptoms and signs together with unequivocally and consistently low testosterone concentrations. It recommends confirmation and evaluation of the cause. Ask your clinician how any existing results were interpreted and whether further testing would change the decision; follow preparation instructions for the exact order.
Keep ED treatment choices individualized
NIDDK's ED treatment overview describes addressing underlying causes when possible and making treatment choices around the person's circumstances. Do not turn an average study result into a promised response for yourself.
If erections remain a concern on TRT, ask whether the original treatment goal is being met, what other contributors need assessment and which options fit your medicines and health history. Do not raise, retime, stop or combine prescribed treatments from an online guide. Adding an ED medicine is a separate prescribing decision, not an automatic next step.
Agree on follow-up before changing the plan
-
What specific improvement are we reviewing, and how will we assess it?
-
Which monitoring is relevant to my treatment and circumstances?
-
What would lead us to reconsider the diagnosis, preparation or treatment?
-
How do fertility plans or other health conditions affect the options?
-
Which symptoms should prompt contact sooner, and whom should I contact?
The 2018 guideline includes fertility plans among factors that can affect whether testosterone treatment should be started. Bring that goal up explicitly. This page does not prescribe a universal laboratory panel, response deadline or stopping schedule.
Bring an optional private note
Record the symptom, when it changed, current prescription instructions, medicines and supplements, and relevant existing results with dates. Keep observations separate from assumed causes. A diary or failed self-treatment trial is not a prerequisite for asking for care. Severe or urgent symptoms require appropriate urgent assessment rather than waiting for a routine hormone review.
